how do i get the transfer form for a domain

I am in a process of transferring my domain womaniko.com . It was deactivated and sitting with a person who no longer is my service provider. He says he has reactivated it and is asking that I send a trasfer code. How do i do that?

I think it's best you sign up with us as a transfer-in. A transfer code is from the registrar, not ISP you wish to move to.
